Oh, I actually took from episode 22 that the problem with his hand is entirely psychological. It's been tearing at him and wearing him down that, during these days which may well be his last with ES, while his woman is dying, instead of staying by her side or finding an antidote for her illness, he's been out and about, killing "pitiful" people (for the king). So basically, CY's entire being is rebelling against him having to carry that sword and slay people with it. While his woman is dying alone. That's why he can't hold the sword anymore.
